Whether you feel a pinch at the top of your backswing or pain during your follow-through, shoulder discomfort is one of the most common issues we see in golfers. With the right approach, most shoulder pain in golfers can be fixed for good. The key is understanding why it happens and addressing the root cause — not just the symptoms. Here are three proven fixes to help you swing freely again and play with the confidence that your shoulder is not going to hurt.
Fix #1: Improve Shoulder Mobility (and Don’t Forget the Upper Back)
A healthy shoulder needs both mobility and stability. This means that the shoulder not only needs to be able to move freely but move in a coordinated and controlled manner. For golfers, limited mobility and control of the shoulder joint, including the shoulder blade, often leads to compensations that cause irritation, pain, and eventually damage. Have you ever felt a pinch during your backswing or follow-through? It’s possible that your shoulder movement may be less than ideal. Frequently, it’s not just your shoulder but also your upper back that’s the problem. When the upper back (thoracic spine) can’t extend, rotate, and laterally flex properly, your shoulder ends up doing extra work to compensate for the deficits. This can also lead to pain and potential damage. Here’s a simple tests: In standing, reach one arm behind your head and down the back like you are trying to scratch between your shoulder blades. Make a mental note of where you touched your back. Repeat on the opposite side. Now perform the test reaching up the back from below, as if trying to scratch the bottom of your shoulder blade. Did one side feel tight or painful during either test? Was the motion on one side more limited than the other? If so, limited mobility may be contributing to the shoulder pain you experience during your golf swing. Mobility Fixes: Exercises and stretches like open books, thoracic segmental extension/rotation, and wall slides can help restore motion. Knowing which exercises are right for you is crucial to restoring shoulder/thoracic mobility and playing golf pain-free.
Fix #2: Strengthen Your Scapular Stabilizers and Rotator Cuff Muscles
Mobility is only part of the equation — strength and control are equally as important to a pain-free golf swing. The small muscles around your shoulder blade, as well as your rotator cuff muscles, are crucial for controlling the shoulder when swinging a golf club. Weakness, tightness, imbalance, or poor sequencing in these muscles can cause the shoulder joint to move improperly, leading to possible irritation to the muscles and sensitive structures in the joint. In golf, that often shows up as: Pain at the top of the backswing Discomfort when finishing the follow-through A dull ache, or even sharp pain, after long rounds or practice sessions Strength Fix: Focus on exercises that train both stability and endurance: Scapular retractions (with resistance bands) External rotator strengthening Prone T’s and Y’s (to activate scapular stabilizers/postural muscles) For the best results, these exercises should be performed in a controlled fashion with feedback from a qualified healthcare professional. Remember, it’s the quality of these motions that matters as you build up the strength and endurance in these muscles. Poor technique can lead to increased irritation and worsening of your pain.
Fix #3: Improve Your Swing Mechanics
Even the strongest and most flexible golfers can experience shoulder pain if their swing mechanics are incorrect. Watch any PGA Tour event and you will see that no two golf swings are the same. However, flaws such as poor release patterns, early extension, and/or an excessively out to in swing plane, amongst others, can cause repetitive strain/trauma to the shoulder over time. Swing Mechanic Fix: The best way to address improper swing mechanics is to work with both a qualified PGA teaching professional as well as a TPI-certified physical therapist. Working to identify your individual physical restrictions can help your golf professional implement swing changes that will work for YOU. Why spend time trying to implement mechanical swing changes that your body is incapable of performing, or worse, is causing you more damage and pain.
